Applicants need to bring the following:
- State of Illinois ID or Driver’s License
- Birth Certificate
- Social Security Card
- Public Aid Letter or Last Year's Tax Return or W2
If under 18, applicants must also bring:
- Parent or Guardian's Last Year's Tax Return or W2
- Secondary School Reference Form
- Withdrawal Letter From the Last School Attended.
